#LI-SA1 Licensed Practical Nurse - Hospice Full-time in Central WI & surrounding areas $29-35/hr, based on experience

Summary/Objective:

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with team members to achieve positive patient care outcomes.

Essential Functions:

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions:

  • Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
  • Implements current nursing practices following a comprehensive assessment and the plan of care.
  • Provides accurate and timely documentation of patient services to reflect the plan of care.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self-care plan.
  • Participates in coordination of hospice services, appropriately reporting identified needs to the interdisciplinary team. Examples include hospice aide, OT, PT, SW, ST, dietitian, or clinical supervisor.
  • Uses and prepares equipment and supplies effectively, adhering to aseptic techniques.
  • Provides appropriate pain and symptom management. Evaluates patient's response to treatments and medications.
  • Participates in personal, professional growth and development, as well as in the organization's quality assessment and performance improvement activities.
Work Environment/Physical Requirements:

The responsibilities involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling, and twisting. The employee must be a licensed driver with a well-maintained and insured automobile. Work conditions may change due to weather and travel requirements, and adaptability is necessary.

Required Education and Experience:
  • Licensed Practical Nurse graduated from an accredited program, currently licensed, and in good standing with the Wisconsin State Board of Nursing.
Preferred Education and Experience:
  • One to two years of nursing experience in an acute care setting and at least one year in home health, hospice, or comparable experience.
  • Interest in hospice philosophy and comfort care.
  • Ability to work cooperatively with other disciplines such as social workers, chaplains, CNAs, and volunteers.
