#LI-SA1 Licensed Practical Nurse - Hospice Full-time in Central WI & surrounding areas $29-35/hr, based on experience
Summary/Objective:
The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the team to achieve positive patient outcomes.
Essential Functions:

- Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
- Implements current nursing practices following comprehensive assessment and the plan of care.
- Provides accurate and timely documentation of patient services to reflect the plan of care.
- Assesses and provides patient and family/caregiver education relevant to diagnosis and self-care plans.
- Participates in coordination of hospice services, reporting needs to the interdisciplinary team, including hospice aides, OT, PT, SW, ST, Dietitian, or Clinical Supervisor.
- Uses and prepares equipment and supplies effectively, adhering to aseptic techniques.
- Provides appropriate pain and symptom management, evaluating patient responses to treatments and medications.
- Participates in personal and professional growth, as well as in quality assessment and performance improvement activities.
Work Environment/Physical Requirements:
This role involves significant physical activities such as standing, lifting, bending, stooping, pushing, pulling, and twisting. The employee must be a licensed driver with a well-maintained and insured vehicle. Adaptability to changing work conditions, including weather and travel, is essential.
Required Education and Experience:
- Licensed Practical Nurse graduated from an accredited program, currently licensed and in good standing with the Wisconsin State Board of Nursing.
Preferred Education and Experience:
- One to two years of nursing experience in an acute care setting, with at least one year in home health, hospice, or comparable experience.
- Demonstrated interest in hospice philosophy and comfort care.
- Ability to work cooperatively with other disciplines including social workers, chaplains, CNAs, and volunteers.
