Concierge Home Care in Jacksonville, FL is hiring a Home Health LPN to provide skilled nursing in patients' homes. This full-time role offers a flexible schedule with autonomy and 1:1 patient care in the Southside and Beaches area.

You’ll use AI-assisted charting and WellSky/Kinnser EMR to document care, receive onboarding, ongoing clinical training, and opportunities for quarterly bonuses. A Florida LPN license and CPR are required, with driving and transportation essential, and benefits

Formación

Active Florida LPN license.
CPR certification.
Reliable transportation with a valid driver's license.
Home health experience is preferred, but not required.

Responsabilidades

Provide skilled nursing in patients' homes under RN supervision.
Perform patient assessments and assist with care planning.
Administer medications and treatments as ordered.
Educate patients and caregivers on disease management and care plans.

Your Role as a Home Health LPN

Provide skilled nursing care under the supervision of a Registered Nurse
Perform patient assessments and assist with care planning
Administer medications and treatments as ordered
