Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) to deliver skilled nursing and one-on-one care in patients’ homes across Clay County, FL.

Formación

• Active Florida LPN license
• Valid driver’s license and reliable transportation
• CPR certification
• Home health experience preferred, but not required

Responsabilidades

• Provide skilled nursing care under the supervision of a Registered Nurse
• Perform patient assessments and assist with care planning
• Administer medications and treatments as ordered
• Educate patients and caregivers on disease management and care plans
• Monitor patient condition and report changes to the supervising RN
• Collaborate with physicians and interdisciplinary teams
• Complete documentation using AI-assisted tools
• Health insurance starts after 30 days

Your Role as a Home Health LPN

Qualifications
