Job summary

Attentive Care NY is actively seeking licensed practical nurses (LPNs) to fill current staffing needs in Albany area facilities. We offer shifts across all day parts, including weekends, with a focus on compassionate, dependable care and reliable staffing solutions to our partner facilities.

Responsibilities include direct resident care, medication administration, wound and catheter care, and collaboration with RNs and therapists. NYS licensure and eligibility to work in the U.S. are required.

Qualifications

• Current New York State LPN license and active registration
• Recent experience in skilled nursing, rehab, or long-term care settings
• Eligibility to work in the United States
• Reliable transportation
• Experience recognizing and reporting changes in resident condition

Responsibilities

• Provide direct nursing care to residents per individualized care plans and orders
• Administer prescribed medications and treatments
• Monitor vital signs and health status, report significant changes
• Perform wound care, dressing changes, catheter care, and related procedures
• Maintain accurate documentation in medical records in compliance with policy
• Supervise and support CNAs, promoting high standards of resident care
• Collaborate with RNs, physicians, therapists for coordinated care
• Respond to emergencies and infection control protocols as required
• Support resident dignity, safety, and independence while communicating with families

These facilities currently have significant LPN staffing needs across all shifts including:

• 7:00 AM - 3:00 PM
• 3:00 PM - 11:00 PM
• 11:00 PM - 7:00 AM
• Weekend availability is especially needed

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans, physician orders, and facility policies.
• Administer prescribed medications, treatments, and delegated nursing procedures accurately and safely while monitoring for effectiveness and adverse reactions.
• Assess, monitor, and document residents' vital signs, physical condition, and changes in health status, promptly reporting significant findings to the Registered Nurse or attending provider.
• Perform wound care, dressing changes, specimen collection, catheter care, and other nursing procedures within the LPN scope of practice.
• Maintain accurate, timely, and complete documentation in the electronic or paper medical record in accordance with regulatory and facility requirements.
• Supervise and support Certified Nursing Assistants (CNAs) by providing direction, monitoring delegated tasks, and promoting high standards of resident care.
• Collaborate with Registered Nurses, physicians, therapists, and the interdisciplinary care team to ensure coordinated, high-quality resident care.
• Respond appropriately to changes in resident condition, medical emergencies, and infection prevention protocols while following established clinical procedures.
• Promote resident dignity, comfort, safety, and independence while building positive relationships with residents, families, and fellow healthcare professionals.
• Adhere to all applicable state and federal regulations, facility policies, infection control standards, and patient confidentiality requirements

Required Qualifications

Preferred Qualifications

• Experience with medication administration, wound care, dressing changes, specimen collection, and catheter care
• Experience documenting care accurately in EMR systems
• Experience supervising CNAs or working closely with RNs, physicians, and therapists
• Availability to work weekend, evening, or overnight shifts
