Attentive Care is seeking licensed practical nurses (LPNs) to join our Capital Region staffing team in Albany, NY. These current opportunities offer multiple shifts including day, evening, overnight, and weekend options to fit your availability.

Ideal candidates hold a New York LPN license, with experience in skilled nursing, rehab, or long-term care, and are able to work with CNAs and RNs to deliver compassionate resident care. Sign-on bonuses and comprehensive benefits are available.

Qualifications

• Current New York State LPN license and active registration.
• Recent experience in skilled nursing, rehab, or long-term care settings.
• Experience recognizing and reporting changes in resident condition.
• Eligibility to work in the United States.
• Reliable transportation.

Responsabilités

• Provide direct nursing care to residents according to care plans and facility policies.
• Administer medications and treatments accurately while monitoring for effectiveness.
• Assess and document vital signs and health status; report significant findings.
• Perform wound care, dressing changes, specimen collection, catheter care.
• Maintain accurate documentation in medical records per regulations.
• Supervise CNAs and support RNs and physicians.
• Collaborate with the care team for coordinated, high-quality care.
• Respond to emergencies and infection prevention protocols.
• Promote resident dignity, safety, and independence.
• Adhere to state/federal regulations and confidentiality requirements.

• Glendale Nursing Home – Schenectady, NY
• Daughters of Sarah- Albany, NY

These facilities currently have significant LPN staffing needs across all shifts, including:

• 7:00 AM – 3:00 PM
• 3:00 PM – 11:00 PM
• 11:00 PM – 7:00 AM
• Weekend availability is especially needed

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans, physician orders, and facility policies.
• Administer prescribed medications, treatments, and delegated nursing procedures accurately and safely while monitoring for effectiveness and adverse reactions.
• Assess, monitor, and document residents’ vital signs, physical condition, and changes in health status, promptly reporting significant findings to the Registered Nurse or attending provider.
• Perform wound care, dressing changes, specimen collection, catheter care, and other nursing procedures within the LPN scope of practice.
• Maintain accurate, timely, and complete documentation in the electronic or paper medical record in accordance with regulatory and facility requirements.
• Supervise and support Certified Nursing Assistants (CNAs) by providing direction, monitoring delegated tasks, and promoting high standards of resident care.
• Collaborate with Registered Nurses, physicians, therapists, and the interdisciplinary care team to ensure coordinated, high-quality resident care.
• Respond appropriately to changes in resident condition, medical emergencies, and infection prevention protocols while following established clinical procedures.
• Promote resident dignity, comfort, safety, and independence while building positive relationships with residents, families, and fellow healthcare professionals.
• Adhere to all applicable state and federal regulations, facility policies, infection control standards, and patient confidentiality requirements.

Required Qualifications

• Current New York State LPN license and active registration.
• Recent experience in skilled nursing, rehab, or long-term care settings.
• Experience recognizing and reporting changes in resident condition.
• Eligibility to work in the United States.
• Reliable transportation.

Preferred Qualifications

• Experience with medication administration, wound care, dressing changes, specimen collection, and catheter care.
• Experience documenting care accurately in EMR systems.
• Experience supervising CNAs or working closely with RNs, physicians, and therapists.
• Availability to work weekend, evening, or overnight shifts.
