Job summary

Attentive Care is seeking qualified LPNs to meet staffing needs across Capital Region facilities. Direct nursing care, med administration, and patient monitoring are core responsibilities for these roles.

Join a team supporting CNAs and collaborating with RNs, physicians, and therapists to ensure high-quality resident care across multiple shifts, including weekends.

Qualifications

• Current New York State LPN license and active registration.
• Recent experience in skilled nursing, rehab, or long-term care settings.
• Experience recognizing and reporting changes in resident condition.
• Eligibility to work in the United States.
• Reliable transportation.

Responsibilities

• Provide direct nursing care to residents per care plans and facility policies.
• Administer medications, treatments, and delegated procedures with safety and accuracy.
• Monitor and document residents' vital signs and health status; report significant changes.
• Perform wound care, dressing changes, and catheter care.
• Maintain accurate documentation in electronic or paper medical records.
• Supervise CNAs and support RNs, physicians, and therapists.
• Collaborate with care team to ensure high-quality resident care.
• Respond to changes in condition, emergencies, and infection control protocols.

• Glendale Nursing Home – Schenectady, NY
• Daughters of Sarah- Albany, NY

These facilities currently have significant LPN staffing needs across all shifts, including:

• 7:00 AM – 3:00 PM
• 3:00 PM – 11:00 PM
• 11:00 PM – 7:00 AM
• Weekend availability is especially needed

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans, physician orders, and facility policies.
• Administer prescribed medications, treatments, and delegated nursing procedures accurately and safely while monitoring for effectiveness and adverse reactions.
• Assess, monitor, and document residents' vital signs, physical condition, and changes in health status, promptly reporting significant findings to the Registered Nurse or attending provider.
• Perform wound care, dressing changes, specimen collection, catheter care, and other nursing procedures within the LPN scope of practice.
• Maintain accurate, timely, and complete documentation in the electronic or paper medical record in accordance with regulatory and facility requirements.
• Supervise and support Certified Nursing Assistants (CNAs) by providing direction, monitoring delegated tasks, and promoting high standards of resident care.
• Collaborate with Registered Nurses, physicians, therapists, and the interdisciplinary care team to ensure coordinated, high-quality resident care.
• Respond appropriately to changes in resident condition, medical emergencies, and infection prevention protocols while following established clinical procedures.
• Promote resident dignity, comfort, safety, and independence while building positive relationships with residents, families, and fellow healthcare professionals.
• Adhere to all applicable state and federal regulations, facility policies, infection control standards, and patient confidentiality requirements.

Required Qualifications
