Job Summary

Under the supervision of a Registered Nurse, the LPN provides quality and cost-effective direct care for patients and families within a unit, service, or other primary area of practice. Collaborates with the health care team to assist patients in achieving established outcomes of care. Reports to the Nurse Manager.

Responsibilities include:

  • Collecting health information and communicating pertinent information to the physician and health care team.
  • Assisting in carrying out the physician’s plan of care, providing health information and education, and coordinating care and community resources as required.
  • Performing basic nursing tasks and patient care in accordance with policies and procedures, including maintaining labs, CGM stats, chart prep, etc.
  • Providing patient care in a non-acute outpatient environment, including the Endocrine Clinic, Diabetes Education program, and other outpatient populations.
  • Supporting select administrative tasks such as patient registration, check-in and check-out procedures, and handling incoming phone calls efficiently and courteously.

  • Current Connecticut LPN license
