Job summary

Hartford HealthCare Medical Group in Southington, CT, is seeking an LPN to deliver direct patient care within a unit or service. The LPN will collaborate with the health care team to support established care plans, document patient data, and report progress to the RN/MD.

This role requires a Connecticut LPN license and prefers prior Medical/Surgical office experience. The position operates Monday through Friday with locations around the state, offering care for diverse specialties and pathways

Qualifications

• Current Connecticut Licensed Practical Nurse license required.
• Medical/Surgical office experience preferred.
• Specialization and/or specific work experience may be required

Responsibilities

• Collaborates with health care team to assist patients in achieving outcomes of care.
• Performs direct patient care and documents data related to the plan of care.
• Under RN/MD direction, performs therapeutic measures to support patient outcomes

Job Summary

Within the context of the Nursing Professional Practice Framework, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice.

• Collaborates with members of health care team to assist patients in achieving established outcomes of care
• Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or MD, to assist in achieving established patient care outcomes
