Job summary

The Hospital of Central Connecticut, part of Hartford HealthCare, is hiring a Licensed Practical Nurse to provide direct patient care in outpatient settings. Under RN supervision, you will collaborate with the care team, collect health information, and support care coordination.

You will assist in implementing the physician's plan of care, educate patients, and handle basic administrative tasks such as registration and check-in/out. Bilingual English/Spanish preferred.

Qualifications

• Current Connecticut LPN license.
• Previous experience in an outpatient, ambulatory or physician practice setting.
• Excellent customer service skills.
• Effective written and verbal communications skills.
• Independent problem solving skills.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide quality, cost-effective direct patient care for patients/families in an outpatient unit or service.
• Collaborate with health care team to achieve established outcomes of care.
• Report to Nurse Manager.
• Collect health information and communicate pertinent details to physician/healthcare team.
• Assist in carrying out physician's plan of care and educate patients.
• Coordinate care and community resources as needed.
• Maintain labs/CGM stats/chart prep as required.
• Handle select administrative tasks including patient registration/check-in/check-out and phone calls.
