Job summary

Elder Homestead Community, part of Volunteers of America National Services (VOANS), seeks an LPN in Minnetonka, MN. The role supports residents with compassionate care, medication administration, and teamwork in a welcoming campus environment.

Qualifications

• Graduate from an accredited school of nursing with current LPN licensure for Minnesota.
• Ability to read and interpret residents’ charts and orders.
• Strong communication with staff, families and outside agencies.
• Demonstrated patient-service orientation and prioritization skills.
• Proficient use of computers for timekeeping, documentation and in-servicing.
• Commitment to resident dignity and quality care.

Responsibilities

• Listen to shift report and maintain open communication with the team.
• Make rounds to observe residents' physical and psychosocial status.
• Assign personnel to meet resident needs on the shift.
• Ensure unit staff and volunteers perform assigned tasks.
• Maintain competence in required skills and documentation practices.
• Provide respectful, person-centered care and protect resident dignity.
• Observe, record and report resident status and medication responses to the supervisor.

Pay Range: $30.15 - $33.30 per hour based on years of experience

Shift: Full Time Monday-Friday 8:00am-4:30pm - occasional weekends

What You Bring (Requirements)

• Graduate from an accredited school of nursing with current licensure as a Licensed Practical Nurse in the state of practice.
• Effective communicator with staff, families, and outside agencies.
• Ability to prioritize duties.
• Customer service-oriented with knowledge of successful customer service techniques.
• Ability to understand residents’ charts, doctor’s orders, residents’ plan of care, and medication orders.
• Must possess the ability to effectively gather clinical data in regard to resident status and condition.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation

What You’ll Be Responsible For

The following duties are normal for this position. This list is not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

• Listen to shift report and keep open lines of communication with supervisor, co-workers and on coming shift.
• Make rounds to residents to observe their physical and psychosocial status.
• Assign personnel appropriately to meet resident needs.
• Ensure that unit personnel and volunteers on your shift are carrying out their assignments
• Maintain competence in all required skills within the facility.
• Treat and ensure that residents are treated with respect and dignity.
• Observes, records, and reports to supervisor regarding residents’ conditions, reaction to drugs and treatments and significant events.
• Maintains all required documentation in an accurate and timely manner according to policies and procedures of the facility, including documentation of medications, treatments, restraints, psychotropic medications and skin condition.
