Job summary

Lifepoint Health seeks an organized Licensed Practical Nurse (LPN) for Centerpoint Family Medicine in Winchester, KY. The LPN will provide direct patient care under an RN, perform assessments, and support daily living activities while ensuring accurate documentation and patient advocacy.

The role requires an LPN license, ADN, and BLS within 30 days of hire, with duties including medication administration per scope and coordination of patient care with the care team.

Qualifications

• Must hold a current LPN license.
• Associate's degree from an accredited nursing program.
• Good standing with the Board of Health Professions.
• BLS certification within 30 days of hire.

Responsibilities

• Provide direct patient care under supervision of a registered nurse.
• Assist with data collection for RN assessment.
• Perform plan of care interventions including specimen collection and wound care.
• Assist patients with activities of daily living and personal care.
• Monitor and document patient status and reactions to interventions.
• May administer medications per scope of practice.

Schedule: Monday - Friday, 8:00a-4:30p

Centerpoint Family Medicine on Boonesboro Road, formerly Clark Clinic Family Medicine, treats children & adults. We provide care for colds, flu and upper respiratory infections, diabetes, hypertension and high cholesterol, ear, throat and sinus infections, annual exams and women's health. We also offer same day sick appointments.

How you'll contribute

A Licensed Practical Nurse (LPN) who excels in this role:

• Provides direct patient care in accordance with applicable scope and standards of practice, as supervised and delegated by a registered nurse,
• Contributes to the assessment of patients by collecting data for analysis by an RN.
• Performs plan of care interventions, including specimen collection and wound care.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
• May administer medication.
• Documents patient care given
• Works as an advocate for the physical and emotional well-being of the patient.
• Schedules appointments and procedures as ordered by the provider.
• Obtains insurance re-authorizations as requested for testing and scheduled procedures.

What we're looking for

Applicants should have a current LPN license and possess an associate's degree from an accredited nursing school. Additional requirements include:

• Good standing with the Board of Health Professions
• Certifications: AHA Basic Life Support (BLS) within 30 days of hire.
• Knowledge of ICD-9, ICD-10, CPT codes, insurance benefits and healthcare background, preferred
