Job summary

Senior Lifestyle Corporation is seeking a Licensed Practical Nurse (LPN) for our Barrington of West Chester community. The position is part-time, with day and night shifts, and focuses on delivering licensed nursing services in accordance with company guidelines.

Responsibilities include medication administration, admissions assessments, developing care plans, clinical documentation, and coordination with physicians and staff.

Qualifications

• Must hold an active LPN license in the state where practicing.
• CPR certification is required.
• 1+ years of experience in senior care or related field.
• Proficient in English to perform duties and communicate with team.

Responsibilities

• Administer medications in compliance with Federal & State regulations and company guidelines.
• Assist with admissions assessments and documentation for new residents.
• Develop and maintain acute and long-term care/service plans.
• Track clinical information (e.g., wounds, infection control) under supervisor direction.
• Provide health-related consultation and communicate changes to physicians and families as needed.
• Respond promptly to emergencies and coordinate with staff across departments.
• Educate residents and staff on health topics and safety practices.
• Maintain confidentiality of residents’ information and ensure accurate documentation.

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
