Legacy Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care and rehabilitation setting. You will administer medications, monitor health status, assist with ADLs, and collaborate with RNs and physicians to support recovery goals.

The ideal candidate holds a current LPN license, BLS/CPR, and demonstrates strong clinical and communication skills with a compassionate, patient-focused approach in a team environment.

Qualifications

• Current and valid LPN license is required.
• Strong clinical skills, including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Compassionate, patient-focused approach to care.
• BLS/CPR certification required.

Responsabilités

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician.
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed.
• Collaborate with the rehabilitation team to support residents’ recovery goals, including physical, occupational, and speech therapy plans.
• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe and clean environment, adhering to infection control protocols.
