Job summary

Springdale Health and Rehabilitation is seeking a skilled Licensed Practical Nurse (LPN) to join our evening shift team. You will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary staff to maintain high standards of care in our long-term care and rehabilitation setting.

This full-time role offers an $10,000 sign-on bonus, competitive hourly pay of $27.00–$32.00 based on experience, evening shift differential,

Qualifications

• Current and valid LPN license.
• Strong clinical skills including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team.
• Compassionate, patient-focused approach to care.
• BLS/CPR certification required.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status, including vital signs.
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding.
• Collaborate with the rehabilitation team to support residents’ recovery goals.
• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe and clean environment, adhering to infection control protocols.
