Roles include administering meds, coordinating admissions, and collaborating with a multidisciplinary team to ensure high-quality resident care.

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Jersey.
• CPR certification required.
• Strong communication and teamwork skills.

Responsibilities

• Administering medications and treatments safely and accurately.
• Coordinating resident admissions, discharges, and transfers.
• Communicating with physicians, pharmacies, residents, and families.
• Monitoring residents' conditions and reporting changes promptly.
• Collaborating with the interdisciplinary care team to provide outcomes.
• Implementing and evaluating individualized care plans.
• Maintaining accurate and timely clinical documentation.
• Following infection control, safety, and emergency procedures.
• Supervising nursing assistants and support staff as appropriate.
• Ensuring resident dignity, privacy, and rights.
• Maintaining knowledge of nursing equipment and proper usage.
