Job summary

Civic Center Health and Rehabilitation is seeking a skilled Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care setting. You will assist with rehabilitation plans and work with RNs, physicians, and the interdisciplinary team to ensure high standards of care.

Qualifications

Responsibilities

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.

• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.


• Provide emotional support and education to residents and their families.


• Ensure a safe and clean environment, adhering to infection control protocols.
