Job summary

Bristol Hospice in Everett, WA is seeking an LPN/LVN to join our team for a full-time day shift. Provide direct patient care, document care plans, and educate patients and families while coordinating hospice services with the interdisciplinary team.

Must hold an LPN/LVN license in the state and have about 3 years of nursing experience, preferably in hospice or medical/surgical settings. Flexible hours and opportunities for growth are available.

Qualifications

• Must be licensed as LPN/LVN in the State
• Minimum 3 years nursing experience, hospice or medical/surgical experience preferred
• Must understand hospice philosophy and needs of the terminally ill
• Must be flexible in work hours
• Willing to maintain knowledge of information systems and software

Responsibilities

• Provide direct patient care per State Nurse Practice Act, including end-of-life duties
• Document patient services accurately and timely reflecting the plan of care
• Educate patients and families about diagnosis and self-care plan
• Coordinate hospice services, reporting needs to the interdisciplinary team
• Manage pain and symptoms and evaluate patient responses
• Participate in ongoing professional growth and organization quality initiatives

Qualifications: An Average Day:

(Includes, but not limited to)

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
