St. Sophia Health & Rehab is seeking compassionate Licensed Practical Nurses to provide quality care for residents with dementia and Alzheimer’s. You will participate in individualized care plans, administer medications, and document patient progress in a team-oriented environment.

Qualifications

• Graduate of accredited LPN School of Nursing.
• One year of nursing experience preferred.
• Valid LPN license.
• Valid CPR certification.
• Working email and phone number for employer communication.
• Ability to work cooperatively as a team member.
• Ability to communicate effectively with residents and families.
• Ability to react decisively in emergency situations.
• Maintain confidentiality.
• Knowledge of training techniques for clinical staff.

Responsibilities

• Initiates and leads individualized nursing care plans.
• Assesses and documents residents’ condition and nursing needs.
• Administers medications and starts IVs as required.
• Implements physicians’ orders accurately and promptly.
• Assigns nursing care to team members per residents’ needs.
• Maintains supplies and equipment in a safe, clean condition.

Available Shifts

• Days 7:00 am - 7:00 pm
• Nights 7:00 pm - 7:00 am

• $12,000 Sign-on Bonus for full-time qualifying candidates!

Qualifications of Licensed Practical Nurse (LPN)

• Graduate of accredited LPN School of Nursing
• One year of nursing experience is preferred
• Valid LPN license
• Valid CPR certification
• Candidates must maintain a working email address and phone number for employer communication.
• Ability to work cooperatively as a member of a team
• Ability to communicate effectively with residents and their family members, and at all levels of the organization
• Ability to react decisively and quickly in emergency situations
