Job summary

Summit Home Care & Hospice is seeking an experienced LPN to provide direct patient care across Milford, OH and surrounding areas. You will work under a registered nurse, follow care plans, and travel between patient visits with occasional remote coordination from the local office.

The role requires an active Ohio LPN/LVN license and at least 3 years of nursing experience, preferably in home health or medical/surgical settings.

Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely

Territory: ~30-Mile Radius (Mileage Reimbursed)

Pay Rate: $28.00 - $31.00 per Hour

Location: Milford, OH & Surrounding Areas

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
• Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse, including caring for hospice patients as census allows.
• Participate in weekend and holiday on-call rotation requirements.

Qualifications

• Must be a graduate from an accredited practical nurse or vocational nursing program.
• Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.
• 3 years of nursing experience, preferred.
• Prior experience in community/home health and/or medical/surgical, preferred.
• Comply with accepted professional standards and practices.
• Demonstrates good verbal and written communication, and organizational skills.
• Must be able to lift, position or transfer patients and lift supplies and equipment.
