Job summary

Nuclear Care Partners is seeking an Licensed Practical Nurse (LPN) for in-home care in Walworth, WI. This part-time night position involves delivering skilled, one-on-one nursing care directly in patients' homes, supported by a nurse-led, mission-driven team focused on dignity and outcomes.

Responsibilities include developing care plans, documenting care and observations, and collaborating with interdisciplinary teams while ensuring compliance with state and federal regulations.

Qualifications

• Current LPN license in Wisconsin.
• Current CPR certification.
• Valid driver's license and reliable, insured vehicle.
• Home health experience preferred but not required.
• Must pass pre-employment screenings.
