Job summary

Westerly Rehabilitation and Healthcare Center in Rhode Island is seeking an LPN for full‑time or part‑time roles on the 7‑3 shift. Daily pay, a $5 PIB, and a $6,000 sign‑on bonus for full‑time ($3,000 for part‑time) are offered.

Hourly pay ranges from $35 to $49, all inclusive. You will provide patient care per care plans, administer medications, monitor residents, document accurately, and collaborate with the interdisciplinary team to deliver compassionate, person‑centered care in a 106‑bed

Qualifications

• Current, unencumbered LPN license in Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care per the written plan of care and physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments.
• Monitor residents’ conditions and report changes promptly to the supervising nurse or physician.
• Maintain accurate, timely documentation in accordance with orders and care plans.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Rhode Island.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
