What You’ll Do:

• Provide skilled nursing care and routine follow-up visits under the supervision of an RN.
• Monitor patient progress and document clinical notes.
• Administer medications, treatments, and wound care as needed.
• Educate patients and caregivers on care plans, safety, and wellness practices.
• Report patient concerns and changes in condition promptly to the supervising RN.

Qualifications

What We’re Looking For:

• Active LPN license in the state of Michigan.
• At least one (1) year of clinical experience; home health experience preferred.
• Strong communication and time-management skills.
• Reliable transportation, valid driver’s license, and auto insurance required.
• Passion for delivering patient-centered care in a home setting.

Compensation:

• Per Visit: $40– $55 (depending on experience)
