Job summary

Ascend Hospice is seeking a Licensed Practical Nurse for the South West Team to provide compassionate patient and family support in a hospice setting. The role involves coordinating care with the Hospice Nurse, attending physician orders, and ensuring continuity of care through strong teamwork and communication.

You will observe patient needs, document changes, and participate in interdisciplinary meetings while maintaining readiness of home care supplies.

Qualifications

• Holds a valid LPN license for the state of operation.
• 1 year of clinical experience preferred.
• Hospice, long-term care or home care experience preferred.
• Strong verbal and written communication; able to work in inter-disciplinary teams.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family.
• Consults with Hospice Nurse regarding patient status and symptom management.
• Delivers care as prescribed by the attending physician and supervised by the Hospice Nurse and IDC team.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and other assigned meetings.
• Maintains home care supplies for patient care management.
• Collects documentation and paperwork as requested by the Hospice Nurse.
• Assists with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides.

• Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels
• Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team
• Monitors and documents patient/family input and response to care
• Maintains continuity of patient care through ongoing communication with families and other health care professionals
• Participates in Interdisciplinary Team Meetings and all other meetings as assigned
• Maintains sufficient and appropriate home care supplies for adequate patient care management
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary
• Participates in orientation of new hospice aides as assigned
