Job summary

Ascend Hospice & Palliative Care KS is hiring a Licensed Practical Nurse to join our Topeka team delivering compassionate, patient-centered home hospice care under supervision of the Hospice Nurse.

The role requires an active LPN license in Kansas, at least one year of clinical experience, and strong communication. LTC/Home Care/Hospice experience is preferred, with opportunities to advance within a supportive interdisciplinary team.

Qualifications

• Hold a valid and current LPN license within the state of operation.
• 1 year of clinical experience.
• Long Term Care/Home Care/Hospice experience preferred.
• Strong verbal and written communication skills.
• Able to work as part of an inter-disciplinary team.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels.
• Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and all other meetings as assigned.
• Maintains sufficient and appropriate home care supplies for adequate patient care management.
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides as assigned.
