Job summary

Brunswick Health Care Center is seeking a compassionate LPN to provide high-quality patient care for our residents. The role includes conducting assessments, coordinating admissions and discharges, wound care, and leading CNAs and CMTs to ensure compliance with facility protocols.

Qualified candidates must hold an LPN licensure and demonstrate empathy and patience. A $10,000 sign-on bonus is offered for LPNs, along with a comprehensive benefits package and paid time off.

Qualifications

• Strong documentation and reporting of changes in patient status.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
