Job summary

Centers Health Care's New Paltz Center for Rehabilitation & Healthcare in New Paltz, NY is hiring Licensed Practical Nurses (LPN) to join our skilled nursing team. The role involves collecting information for admissions, monitoring vitals, administering medications, wound care, and supporting CNAs as needed.

Qualifications

• Must be able to work as a team member.
• Valid NY State LPN license.
• In good standing with State Registry.

Responsibilities

• Collect information from new Residents to be admitted.
• Record health details of Residents; including vitals & temperature.
• Administer medications and injections to Residents as needed.
• Treat wounds and bedsores as needed.
• May supervise Certified Nursing Assistants (CNAs).
• Help Residents get dressed and take care of personal hygiene.
• Monitor Residents’ food and liquid intake and output.

• Collecting required information from new Residents to be admitted
• Recording health details of Residents; including vitals & temperature
• Administering medications and injections to Residents as needed
• Treating and dressing wounds and bedsores as needed
• May be required to supervise Certified Nursing Assistants (CNAs)
• Helping Residents get dressed & take care of personal hygiene
• Monitoring Residents’ food and liquid intake and output
