Ascend Hospice in Topeka, KS, seeks a compassionate Hospice Registered Nurse Case Manager (RN) to join our team. The role involves on-call duties, case management, and patient visits to ensure comprehensive coverage and coordinated care.

You will assess patients and families, develop and adjust care plans, and collaborate with the care team to deliver high-quality end-of-life support. A valid RN license and willingness to travel are required.

Qualifications

• Associate's Degree in Nursing from an accredited program.
• Must hold a State Certified RN license.
• 2 years' nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation; willingness to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessments of patients and families to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs and adjust care plans.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to direct the work of the team to provide continuous and well-coordinated care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with physicians regarding patients' needs and changes in conditions.
• Supervise Hospice Aides.

We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager (RN) to join our team at Ascend Hospice in Topeka, KS. About the Role

As a Hospice Registered Nurse Case Manager (RN), you will be responsible for assisting the team with on-call, case management, and visits to ensure adequate coverage and call-outs. Your duties will include but are not limited to:

• Completing initial assessments of patients and families to determine hospice care needs
• Regularly re-evaluating patients' nursing needs
• Using assessment data to determine care plan needs
• Collaborating with the healthcare team, patient, and family to direct the work of the team to provide continuous and well-coordinated care
• Counseling patients and families in meeting end-of-life needs
• Initiating appropriate preventative and palliative nursing procedures
• Communicating with physicians regarding patients' needs and reporting any changes in patients' conditions
• Supervising Hospice Aides

Requirements

• Associate's Degree in Nursing
