Job summary

Affirmed Home Care is seeking a compassionate Licensed Practical Nurse (LPN) for Port Jefferson, NY, with 1+ year acute care experience and active NY license. Schedule: Tue–Sat, 1:00 PM–6:00 PM. Pay: $40.00 per hour. Client needs include straight catheter care and skilled nursing services.

Per diem role with weekly direct deposit, sign-on and referral bonuses, and streamlined onboarding. Must have BLS and be eligible to work in the U.S.; driver’s license preferred.

Qualifications

• Active NY State LPN license required.
• Minimum 1 year of acute care nursing experience required.
• Current BLS certification.
• Eligibility to work in the U.S. stated.
• Physical exam within the past 12 months.
• MMR immunization within the past 10 years.
• PPD or QuantiFERON within the past year.
• Chest X-ray within the past 5 years.

Responsibilities

• Provide one-on-one skilled nursing care for a client in Port Jefferson, NY.
• Perform straight catheter care and other nursing tasks as prescribed.
• Coordinate with client and clinical team to ensure quality care.

Education

Active New York State LPN license

Minimum 1 year of acute care nursing experience

Current BLS certification

Eligibility to work in the U.S.
