Job summary

Affirmed Home Care is seeking compassionate Per Diem Licensed Practical Nurses (LPNs) to provide high-quality, individualized in-home care to clients across Westchester County.

We offer flexible 12-hour day and overnight shifts with competitive pay of $40.00 per hour, allowing you to build a schedule that fits your life while delivering one-on-one patient care and meaningful support to families.

Qualifications

• Active New York State LPN license.
• Minimum 1 year of LPN experience in an acute care setting.
• Home care experience preferred.
• Current BLS certification.
• Eligible to work in the United States.
• Physical exam completed within the past year.
• PPD/QuantiFERON or chest X-ray within specified periods.
• MMR immunizations within the past 10 years.
• Driver's license preferred.

Responsibilities

• Provide high-quality, individualized in-home care to clients.
• Support flexible 12-hour day and overnight shifts.
• Deliver one-on-one patient care in clients' homes.
• Collaborate with families and coordinators to meet care plans.
