Affirmed Home Care, New York’s premier concierge home care agency, seeks experienced LPNs to provide highly personalized, one-on-one nursing care throughout Suffolk County. Join a team that values compassionate, dependable care with flexible per diem scheduling.

We offer shifts ranging 6–12 hours in multiple communities with competitive compensation, sign-on and referral bonuses, and driving incentives to support quick placement on cases.

Qualifications

• Minimum of 1 year of current LPN experience.
• New York State LPN license is required.
• BLS certification is required.

Responsibilities

• Provide one-on-one LPN care to clients in their homes.
• Work flexible 6–12 hour shifts across Suffolk County.
• Travel to Huntington, Smithtown, Kings Park and surrounding areas.

The Opportunity

We are seeking compassionate and dependable LPNs for highly specialized cases, with flexible 6–12 hour shifts available throughout Suffolk County.

Compensation

• $40.00 per hour for hourly shifts


• Flexible per diem scheduling

Requirements

• Minimum of1 year of current LPN experience


• Home care experience is a plus


• ValidNew York State LPN License


• ValidBLS Certification


• Valid driver’s license preferred


• Eligible to work in the United States


• Physical exam completed within the past year


• PPD or QuantiFERON completed within the past year, or chest X-ray within the past 5 years


• MMR immunizations within the past 10 years
