Position Summary

Provides primary care to patients in collaboration with the provider (NP and/or MD). Must be flexible to organizational needs, including working in various areas. Functions as part of the interdisciplinary team under the direction of Nurse Practitioners or Medical Doctors.

Essential Duties/Responsibilities

• Performs duties under the supervision of a nurse practitioner or medical doctor.
• Administers medications, vaccinations, and inoculations following established procedures and standards.
• Participates as a member of the interdisciplinary team by carrying out treatment interventions within training and licensure limits and communicating patient observations to team members.
• Documents all care and patient responses in medical records as per procedures and standards.
• Conducts patient/family education on medications and health needs.
• Participates in Quality Improvement activities as assigned.
• Maintains professional behavior aligned with customer relations, teamwork, and federal guidelines.
• Maintains confidentiality per HIPAA policies.
• Participates in staff meetings and continuing education to stay current in nursing practices.
• Works flexibly as organizational needs dictate.
• Maintains basic computer skills for patient recordkeeping.
• Performs other duties as assigned.

Minimum Qualifications

• Knowledge of basic nursing care and vaccination procedures.
• Good communication skills, both oral and written.
• Basic computer skills.

Certificates, Licenses, Registrations

• Licensed Practical Nurse by the State of Indiana.
• Current CPR/AED certification from the American Red Cross.
• Crisis Prevention Intervention (CPI) certification.
• Valid Driver’s License per HCI policy.
• Complete all required continuing education on time.

Physical Demands

Must meet physical requirements to perform essential functions, including exposure to patients with varying mental/emotional states, physical strength, and mobility.

Work Environment

Work is performed in an office setting, requiring good vision and hearing, manual dexterity, and occasional lifting of up to 25 pounds.
