Job summary

Stone Rehab and Senior Living is seeking a Licensed Practical Nurse (LPN) for full-time overnight shifts in Tallahassee, FL. The role involves administering medications, monitoring vitals, and delivering prescribed treatments per care plans.

You will support residents with ADLs, document clinical status in the EHR, and collaborate with the interdisciplinary team while upholding safety and infection control standards. A $5,000 sign-on bonus is offered.

Qualifications

• Active LPN license in MA and current CPR certifications (BLS/ACLS).
• Strong clinical skills in medication administration, vitals, and documentation.
• Knowledge of long-term care regulations and patient safety protocols.
• Fluent English communication, both spoken and written.

Responsibilities

Delivering Solutions that Matter:

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications for Success:

• Active, unencumbered MA Licensed Practical Nurse (LPN) licensure
• Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR).
• Knowledge of long-term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting.
• Current CPR certifications, including BLS and ACLS.
• Ability to communicate fluently in English - both verbally and written
