Job summary

Stone Rehab and Senior Living is seeking a Licensed Practical Nurse (LPN) to join our care team. The role involves administering medications, monitoring vital signs, and performing prescribed treatments according to physician orders and care plans.

You will assist residents with activities of daily living, support mobility and independence, and collaborate with interdisciplinary teams to maintain continuity of care while upholding infection control and safety standards.

Qualifications

• Active MA LPN licensure.
• Experience in long-term care or skilled nursing.
• CPR certifications (BLS and ACLS).
• Proficient in documenting in electronic health records (EHR).

Responsibilities

• Provide direct resident care including medications administration and monitoring.
• Assist residents with ADLs while promoting dignity and independence.
• Monitor and document clinical status, changes in condition and treatment responses.
• Collaborate with care teams to implement and update care plans.
• Ensure infection control, safety standards and facility policies are followed.

Licensed Practical Nurse | Stone Rehab and Senior Living
$42.00 - $48.00/hour, based on experience
Full-Time | Part-Time | PRN

Delivering Solutions that Matter:

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications for Success:

• Active, unencumbered MA Licensed Practical Nurse (LPN) licensure
• Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR).
• Knowledge of long-term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting.
• Current CPR certifications, including BLS and ACLS.
• Ability to communicate fluently in English - both verbally and written
