Job summary

Signature HealthCARE in Spring City, TN is seeking an LPN/LVN to provide direct nursing care, administer medications, assist with ADLs, and support a compassionate, patient‑centered care team in a skilled nursing facility.

You will supervise nursing assistants, document care using the EHR, and collaborate with physicians and the interdisciplinary team to develop individualized care plans. Weekend and shift differential opportunities enhance compensation.

Qualifications

• Active LPN/LVN license in TN
• Current CPR/BLS certification preferred
• Minimum 1 year of clinical nursing experience; LTC experience preferred
• Experience in a supervisory/charge nurse role preferred
• Strong documentation, observation, and communication skills
• Knowledge of Federal and State long-term care regulations
• Familiarity with electronic health records (EHR) systems

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care
• Assist residents with activities of daily living (ADLs) while ensuring comfort and safety
• Supervise nursing assistants and medication aides during shifts according to policy
• Monitor residents and report changes to physicians; document accurately per policy
• Develop and review individualized resident care plans with the interdisciplinary team
• Coordinate admissions, transfers, and discharges per procedures
• Participate in quality assurance and regulatory surveys as needed
• Perform additional duties as assigned by the Director of Nursing

How You Will Make a Difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What You Need To Make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
