Job summary

Signature HealthCARE in Memphis, TN is seeking a compassionate LPN to deliver direct nursing care to residents, administer medications, assist with ADLs, and supervise nursing assistants. The role requires an active LPN/LVN license in the state, CPR/BLS preferred, at least 1 year of clinical nursing experience, and experience in long‑term care and supervisory/charge nurse roles.

Familiarity with EHR and MS Office is a plus.

Qualifications

• Active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long‑term care preferred.
• Supervisory or charge nurse experience preferred.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long‑term care regulations.
• Effective verbal and written communication; ability to work independently.
• Familiarity with electronic health records (EHR) systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents’ status and notify physician of changes.
• Develop, implement, and review individualized care plans with the team.
• Complete clinical documentation per policy and procedure.
• Coordinate admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities and staff education.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long‑term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long‑term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
