Job summary

Signature HealthCARE is seeking a dedicated LPN to join our Lexington area facility. The role focuses on direct resident care, medication administration, wound care, and adherence to care plans within a supportive, team-oriented environment.

You will supervise nursing staff, ensure accurate documentation, and participate in quality assurance and regulatory activities. A current LPN license in Kentucky and related experience are required, with CPR/BLS preferred.

Qualifications

  • Current, active LPN/LVN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one year of clinical nursing experience; long-term care experience preferred.
  • Experience in a supervisory or charge nurse role preferred.
  • Knowledge of Federal and State long-term care regulations.
  • Effective verbal and written communication skills.
  • Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with ADLs and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
  • Monitor and observe residents’ physical and emotional status; notify physician and responsible party of any change.
  • Develop, implement, and review individualized resident care plans with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy and procedure.
  • Coordinate resident admissions, transfers, and discharges per established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.

How You Will Make a Difference
