Job summary

Signature HealthCARE of Primacy in Memphis, TN, is seeking a compassionate LPN/LVN to provide direct nursing care to residents, administer medications, perform wound care, and support skilled nursing procedures for a 120-bed facility. You will supervise nursing assistants and medication aides, monitor residents, and coordinate admissions, transfers, and discharges with an interdisciplinary team.

Strong documentation, EHR proficiency, and a commitment to person-centered care are essential.

Qualifications

  • Current, active LPN/LVN license in the state of practice.
  • Minimum one year of clinical nursing experience; long-term care experience preferred.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Experience in a supervisory or charge nurse role preferred.
  • Strong clinical observation, medication administration, and documentation skills.
  • Knowledge of Federal and State long-term care regulations.
  • Effective verbal and written communication skills; ability to work independently and prioritize tasks.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
  • Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
  • Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy and procedure.
  • Coordinate resident admissions, transfers, and discharges according to established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.

How you Will make a difference
