Job summary

The Emerald Peek Rehabilitation and Nursing Center is seeking a Licensed Practical Nurse (LPN) for our skilled unit and long-term care unit in New York. The role focuses on medication administration, vital signs, and direct resident care with outcome monitoring.

Applicants must hold a current LPN license in NY. Excellent communication with care teams, residents, and families is essential; CNAs will be supervised by the LPN.

Qualifications

• Current LPN license in good standing (New York).
• Experience in skilled nursing or long-term care is preferred.
• Strong communication and documentation skills.

Responsibilities

• Medication administration and monitoring of outcomes.
• Take and record resident vital signs accurately.
• Provide direct resident care and treatments with outcome monitoring.
• Educate residents and their families about care plans.
• Communicate with physicians, NPs, families and other health professionals.
• Supervise CNAs and support care team.

• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

Salary: Up to $43 an hour
