Aegis Healthcare is hiring a Home Health and Hospice Licensed Practical Nurse (LPN) in Scottsdale, AZ. This full-time role supports patient care at home, following physician plans of care under RN supervision.

Expected duties include nursing treatments, pain management, ADL assistance, and medication administration. You will maintain records, liaise with RNs/physicians, and participate in team meetings to ensure coordinated care.

Qualifications

• Current Licensed Practical Nursing License in state of practice.
• 1+ years of Home Health & Hospice Experience preferred.
• Ability to work independently, be self-directed, and flexible.
• Ability to lift 35lbs.
• Effective verbal & written communication & time management skills.
• Valid Driver's License (clean record) & reliable transportation.
• Proof of current auto insurance & registration.
• Valid AZ Fingerprint Clearance Card (we can help)
• Current CPR Certification (we can help) & Negative TB Test (we can test)

Responsabilités

• Verify and execute the plan of care as prescribed by physicians and under the guidance of a Registered Nurse.
• Provide essential nursing treatments and address patient pain.
• Assist with activities of daily living and educate patients and caregivers on self-care techniques.
• Handle medication administration and ensure availability of necessary supplies.
• Maintain accurate patient records and liaise with supervising RNs or physicians on assessments.
• Contribute to team meetings to coordinate patient care.

COMPENSATION: $34-$38 per hour DOE

STATUS: Full-Time employee

SCHEDULE: M-F 8am to 5pm
