Job summary

Cape Heritage Rehabilitation & Health Care Center in Sandwich, MA is seeking an LPN to provide direct nursing care under RN supervision. You will oversee day-to-day nursing activities, maintain care plans, and ensure high-quality resident care on the 11p-7a shift with weekend rotation.

We offer a sign-on bonus up to $5,000, competitive pay ($40–$46/hour), full-time and part-time options, per diem on all shifts, and benefits including health, dental, vision, and 401(k). New grads welcome.

Qualifications

• Graduate of an approved LPN/LVN program.
• CPR certification required.
• Active and unencumbered LPN/LVN license in Massachusetts, or in process of obtaining one.
• New graduates welcome!

Responsibilities

• Meet the nursing and rehabilitation needs of each resident.
• Maintain current resident care plans on all residents under your care.
• Follow physician's orders and render/assist with complete nursing care, treatments, dressings, and other physician orders.
• Transcribe physician's orders promptly and accurately.
• Review patient records daily to assure accuracy and completeness.
• Document comprehensive and complete nursing notes.
• Visit each resident daily to evaluate resident needs.
• Maintain accurate awareness of residents on alert list.
• Document and report any unusual or significant findings; contact the physician when necessary.
• Administer medication according to procedure.
• Initiate any and all emergency procedures as necessary.
• Demonstrate correct safety techniques.
• Respect and take care of facility equipment and not wasteful of supplies.
• Contribute to effective communication and pleasant working conditions.
• Contribute to and promote positive resident and family relations.
• Comply with facility Privacy Practices Policies & Procedures, protecting residents' individual health information

Company: Cape Heritage Rehabilitation & Health Care Center in Sandwich

Up to a $5,000 sign-on bonus available!* Hourly rate ranges between $40 and $46, depending on the shift. Evenings, overnights, and weekends have shift differentials.

Full-time and part-time available on 11p-7a; includes an every-other-weekend rotation.

Per diem available on all shifts.

Job Summary

• Ability to read, write, speak, and understand the English language




• Ability to make independent decisions when circumstances warrant such actions

• Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care




• Leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel
