Bay Harbor Post Acute and Healthcare Center is hiring a Licensed Practical Nurse (LPN) to join a compassionate, patient-centered team in Maryland. The role supports residents’ physical and emotional needs and works within established care plans and medication protocols.

FT/PT/Per-Diem opportunities available with 8- and 12-hour shifts, daily pay options, and shift differentials. A positive, collaborative environment emphasizes growth, balance, and high-quality care.

Qualifications

• Current, unencumbered active LPN license in Maryland.
• Valid BLS Certification (CPR) required.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment, communication, and attention to detail.

Responsibilities

• Provide patient care per the plan of care and physician orders.
• Follow individualized care plans developed by Nurse Manager and team.
• Administer medications and prescribed treatments per policy.
• Monitor residents, report changes promptly to supervisor or physician.
• Maintain accurate documentation per orders and standards.

Full-Time, Part-Time, & Per-Diem Opportunities Available!

8 HOUR SHIFTS (FT/PT): 7:00AM - 3:00PM, 3:00PM - 11:00PM, Or 11:00PM - 7:00AM

12 HOUR SHIFTS (FT): 7:00AM - 7:00PM Or 7:00PM - 7:00AM

Weekend Only SHIFT (FT): 7:00AM - 11:00PM

Hourly Range: $31.00 – $40.00/hr (Based On Experience & Incentives) ALL INCLUSIVE

No Frills Program For Full-Time Employees: If you opt for no benefits (no vacation, sick time, personal days, or holidays), you’ll earn an additional $3/hour on top of your base pay.

Shift Differentials ($2.50 For 3pm -11pm & $2.00 For 11pm - 7am)

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Maryland.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
