Job summary

York Post Acute is seeking a Licensed Practical Nurse (LPN) for full-time, part-time, and per-diem opportunities. Shifts run 7:00 AM–3:00 PM, 3:00 PM–11:00 PM, or 11:00 PM–7:00 AM with an all-inclusive hourly range of $28.50–$33.50 and single-site shift differentials.

Qualifications

• Current, unencumbered LPN license in Virginia.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with accepted standards of nursing practice and the physician's plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per the plan of care and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond to clinical needs.
• Maintain accurate, timely documentation in accordance with orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Hourly Range: $28.50 - $33.50/hr (Based on Experience & Incentives) ALL INCLUSIVE

Shift Differentials $1.50 For 3pm 11pm & $1.00 For 11pm - 7am)

No Frills Program For Full-Time Employees: If you opt for no benefits (no vacation, sick time, personal days, or holidays), you’ll earn an additional $3/hour on top of your base pay.
