Job summary

Willow Ridge Rehabilitation & Living Center in Rutherfordton, NC is seeking compassionate LPNs to join our skilled nursing and rehabilitation team. We offer a $13,000 sign-on bonus, full-time and part-time positions, and 12-hour day and night shifts to support patient-centered care.

As an LPN, you will administer medications, monitor residents, document care, and collaborate with RNs, CNAs, and physicians to ensure high-quality outcomes.

Qualifications

• Graduate of an approved Practical Nursing program.
• Current and active North Carolina LPN license.
• Current CPR/BLS certification.
• Long-term care, skilled nursing, or rehabilitation experience preferred.
• Strong communication, organizational, and clinical skills.
• Compassionate, dependable, and team-oriented approach to resident care.

Responsibilities

• Provide compassionate, high-quality nursing care in accordance with individualized resident care plans.
• Administer medications and treatments accurately and timely, within the LPN scope of practice.
• Monitor residents for changes in condition and promptly communicate concerns to the appropriate healthcare professionals.
• Collect and document resident health information and contribute to nursing assessments and care planning.
• Complete accurate and timely clinical documentation using electronic health records (EHR).
• Collaborate with registered nurses, certified nursing assistants (CNAs), physicians, and interdisciplinary team members.
• Provide guidance and support to CNAs and other nursing staff as appropriate.
• Follow infection control practices, safety procedures, and applicable state and federal regulations.
• Communicate effectively with residents, families, and healthcare professionals.
• Promote a safe, respectful, and resident-centered care environment.

LPN Responsibilities
