Job summary

Affined Home Care is seeking an LPN to provide in-home nursing care across Princeton and surrounding areas. Travel to clients’ homes, monitor vitals, and document care with confidentiality are key responsibilities.

The role supports personal care, meal prep, and advanced nursing duties including chest tubes, colostomies, and urinary catheters, with emotional support for clients and families.

Qualifications

• Valid LPN license.
• Recent annual physical.
• PPD or QuantiFeron test within the past year, or chest X-ray within five years.
• MMR labs documented.

Responsibilities

• Travel to clients' homes to deliver nursing care based on best practices.
• Check and monitor vital signs for ongoing assessments.
• Document care activities in a thorough, confidential manner.
• Evaluate patients’ well‑being and communicate needs to the Director of Clinical Nursing or emergency services.
• Assist with personal care and meal preparations.
• Provide advanced nursing care, including management of chest tubes, colostomies, and urinary catheters.
• Offer emotional support to clients and their families.
