Job summary

Portageville Health Care Center is seeking a dedicated LPN for a full-time night shift to support residents with compassionate care. The role includes assessing patients, following protocols, and documenting changes to ensure high-quality outcomes.

Responsibilities include admissions, discharges, wound care, and leading CNAs/CMTs while adhering to facility policies and emergency responses. A licensed practical nurse is required and previous long-term care experience is beneficial.

Qualifications

• LPN licensure/certification required.
• Caring, empathetic approach to residents and team.
• Willingness to work night shifts and follow facility protocols.

• Perform admission duties: complete assessment, MAR, TAR, POS, chart setup, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician/guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility policies.
• Respond effectively to emergency situations.
