Job summary

Crest Pointe Rehabilitation and Healthcare Center is seeking a Licensed Practical Nurse (LPN) to join a compassionate team in a 118-bed sub-acute and long-term care setting in New Jersey. Per diem and full-time opportunities are available with shifts from 7 AM-3 PM, 3 PM-11 PM, and 11 PM-7 AM.

Responsibilities include patient care, following care plans, medication administration, and accurate documentation.

Qualifications

• Current, unencumbered active LPN license in NJ.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with standards of nursing practice and plan of care.
• Follow and implement individualized care plans developed by the Nurse Manager and team.
• Perform medication administration and prescribed treatments as per plan and policies.
• Monitor residents’ conditions, report changes promptly to supervisor and physician.
• Maintain accurate, timely documentation per orders, care plans and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Per Diem: 1 Weekend/Month Required

Pay: $31.00-$38.00/hr (All-Inclusive, Based on Experience)

+$1/hr Evening & Night Shift Differential

• Support a safe, respectful, and therapeutic environment for residents and their families.
