The Palms at Plantation is seeking a compassionate Licensed Practical Nurse (LPN) to join our resident care team in Plantation, FL. You will provide individualized nursing care and support residents to maintain independence in a senior living setting.

Qualifications

• Active Florida LPN license in good standing.

Responsibilities

• Provide direct nursing and resident care per individualized plans.
• Administer medications per standards and policies.
• Complete medication documentation and follow-up.
• Conduct resident assessments as required by state regulations and policy.
• Monitor residents and report changes to supervisors.
• Document observations and care activities by end of shift.
• Assist in developing and maintaining individualized care plans.
• Communicate with residents, families, physicians, and care team.
• Guide and support caregivers as appropriate.
• Promote dignity, independence, safety and quality of life.

As an LPN at The Palms at Plantation, you'll play an important role in supporting the health, safety, and well-being of our residents while helping them maintain their independence and quality of life.

What You'll Do

• Provide direct nursing and resident care services according to each resident's individualized plan of care.
• Administer medications in accordance with standards of practice, Florida regulations, and community policies and procedures.
• Complete all required medication documentation, reporting, and follow-up.
• Perform resident assessments as required by state regulations and community policy under the direction of nursing leadership.
• Monitor residents for changes in physical, emotional, or psychosocial condition and immediately report concerns to the appropriate supervisor.
• Document relevant observations, clinical procedures, resident care, and communication with residents' responsible parties by the end of each shift.
• Assist with the development and maintenance of individualized resident care plans.
• Communicate effectively with residents, families, physicians, pharmacies, and other members of the care team.
• Provide guidance and support to caregivers and other resident care associates as appropriate.
• Promote resident dignity, independence, safety, and quality of life.
• Follow infection control, safety, emergency preparedness, and resident care procedures.
• Maintain a positive, professional, and team-focused approach to resident care.
• Perform other duties as assigned.

What We're Looking For

• Current Florida Licensed Practical Nurse (LPN) license in good standing required.
• High school diploma or equivalent (GED) required.
• Current CPR certification required.At least 1 year of related nursing experience preferred.
• Senior living, assisted living, memory care, or long-term care experience is a plus.
• Strong communication, organizational, and clinical skills.
• Compassionate and resident-focused approach to care.
• Dependable and able to work effectively both independently and as part of a team.
• Ability to meet the physical requirements of the position, including assisting with resident transfers and repositioning using appropriate techniques and equipment.
