CenClear is seeking a compassionate Licensed Practical Nurse (LPN) to join our Behavioral Health Division. This role supports wellness and behavioral health services across Clearfield and Philipsburg, PA with occasional travel to other CenClear sites.

The position offers full-time, hourly pay and opportunities to collaborate with a multidisciplinary team. You will provide wellness coaching, assist psychiatric prescribers, collect required data, and participate in trainings to enhance client

Responsabilités

• Primary coverage at Clearfield and Philipsburg locations.
• Four 10-hour days schedule; travel to other CenClear locations as needed.
• Participate in CenClear, CCBHC and wellness/health trainings.
• Provide wellness coaching and related training to staff and clients.
• Coordinate with healthcare providers for continuity of care; facilitate referrals.
• Collect data for outcomes measurement and document appropriately.
• Complete paperwork accurately and in a timely manner.
• Attend monthly supervision.

What You'll Do:

• Work 40 hours per week, 52 weeks per year
• Primary coverage required at both Clearfield and Philipsburg locations.
• Current schedule is four 10-hour days (subject to change based on program needs)
• Occasional travel to other CenClear locations for additional coverage as needed.
• Participate in all required CenClear, CCBHC and wellness/health related education and trainings
• Provide wellness coaching and related training to staff, individuals in services and community partners
• Promote a culture of health and wellness through ongoing participation in meetings and community outreach
• Collaborate and coordinate with healthcare providers for continuity of care
• Facilitate referrals, as needed
• Collect required data for outcomes measurement and contract requirements and document appropriately
• Submit all necessary administrative paperwork according to procedures
• Document all wellness coaching sessions and other wellness/integration activities
• Assist psychiatric prescribers in providing treatment to clients, collecting labs, reports, information, and vitals for provider
• Follow up with clients per prescriber request
• Willingness to learn and adapt to new information and skills and engage in ongoing education, training, and meetings.
• Self-motivation and ability to work independently as well as a team.
• Empathy and understanding in dealing with clients and families.
• Update client history and changes at each appointment.
• Maintain professionalism and confidentiality
• Complete paperwork accurately and in a timely manner.
• Attend and participate in monthly supervisions.

What We're Looking For:

• Licensed Practical Nurse (LPN)
• Experience (two years preferred) in mental health and substance use services.
• Maintain professional and confidential attitude.
• Strong communication and interpersonal skills.
• Attention to detail and accuracy in documentation.
• Flexibility to travel and work varied hours.
• Ability to gather and analyze health-related information.
• Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
