Fox Subacute in Philadelphia, PA is seeking a clinically strong LPN to join our 53-bed skilled nursing and subacute unit, delivering resident-centered care across 12-hour shifts.

Work with physicians, RNs, respiratory therapists, and CNAs to assess and modify care plans, administer medications, respond to emergencies, and document changes with PointClickCare. We offer supportive teamwork and opportunities to advance clinical skills in a high-acuity setting.

Qualifications

• Active Pennsylvania LPN license (required).
• Current BLS/CPR certification.
• Ability to work 12-hour shifts, including nights, weekends, and holidays as needed.
• Strong communication and critical thinking skills.
• Ability to perform physical job duties, including lifting up to 50 pounds.
• English proficiency preferred.
• Ability to commute or relocate to Philadelphia, PA 19145.

Fox Subacute South Philadelphia is a 53-bed skilled nursing and subacute facility specializing in the care of clinically complex respiratory and ventilator-dependent residents. Our environment offers the acuity level of a hospital step-down unit while allowing nurses to build long-term relationships with residents and deliver truly personalized care.
